Active Service Model

The Department of Health’s Active Service Model is a quality improvement initiative that promotes:

  • Person centred care
  • Capacity Building
  • Restorative  Care

 

The principles under pinning this approach are that:

  • People wish to remain autonomous
  • People have the potential to improve their capacity
  • People’s needs should be viewed in an holistic way
  • HACC services should be organized around the person and his or her carer, that is, the person should not be slotted into existing services
  • A person’s needs are best met when there are strong partnerships and collaborative working relationships between the person; their carers and family; support workers and between service providers.

 

This health promoting philosophy of care will require service changes, so that clients are supported to gain the greatest level of independence they can and be actively involved in making decisions about their life.

 

Put simply, the theme is “doing with” not doing “for” people, and encouraging older people to reach their full potential
